You may register with us as an NHS patient if you normally live in the practice boundary area.  Our boundary currently covers the SW11 area only.

Registering is quick and easy

1) Book a new patient appointment 
A one-off appointment with a health care assistant is required so that we have as much information about your health as possible. The appointment will take no more than 25 minutes. Call or email us today to book your registration appointment.

2) Complete our registration forms
Please complete our new Patient Registration Form and Adult New Patient Health Questionnaire prior to your registration appointment.

These registration forms can be used for patients aged 17 and over. For registration of a patient 16 years old and under please visit the Junction Health Centre for a 16 and under registration form.

3) Produce proof of address and identification.
We require proof of identification and address in order to complete your registration. Please bring this with you when you attend your registration appointment.
